Loved grandmother of Jackie and Jo; Nathan, Garth and Kristian; Tamra, Brandon and Kaneisha. Great Nan Phyl to nine.

Family and Friends are invited to attend a funeral Service for the late Mrs Phyllis Maude Monk to be held at the Uniting Church, Lloyd Street, Nyah West on FRIDAY (May 5th 2017) commencing at 11:00a.m. Followed by a Burial at Nyah District Cemetery
